1. File a Medical Claim

Submit a Claim by mail

• It's easy. Just download the claim form below.

• Fill out the first page with:
  • Your personal information
  • Dates of injury or sickness
  • The cause of the injury or date of first symptoms for a sickness
  • Sign and date
  • Attach all receipts

• Please be sure to fill out the entire page.
    Leaving items blank can cause delay in your claim processing.

    • When sending information:

      A.Clip,do not staple,all bills to the completed form.

      B.Make sure all bills or itemized receipts indicate:
    • A diagnosis code
    • Procedure code
    • Date of service
    • Charge
    • The provider's tax ID number

    Submit a Claim Electronically

      • Your doctor also can submit a claim electronically, using Emedeon(formerly WebMD).

      • This gives a faster turnaround time than submitting a claim by mail.

      • Check with your doctor to see if they are a participant. If they are,show them your ID Card.

      • The Emedeon information is printed on the back of every ID Card.

    Submit a Claim
    To file an appeal, please include the following information:
    • A letter requesting an appeal to your claim(s), including your: Name, Address, Phone number, United Health Care Student ID number, Date of service for your injury/sickness, Claim number(s) (located on the top of your Explanation of Benefits), and Email address
    • A copy of your Explanation of Benefits for the claim(s) in question.
    • Medical Records including all test results from all providers visited for the specific injury/sickness that you are appealing.
    Once we receive the documentation, your appeal will be reviewed and a written response will be mailed to you. The response will include what the findings were, whether the appeal was approved or denied, and the reason for the final decision.

    If you are a student and would like to check on the status of a claim that you or a provider submitted, you will need to set up a My Account if you have not done so already. Please visit our My Account Center to log in to an existing account or to create a new one.

    【International Student Medical Insurance】provides maternity benefits. Gold and Elite Plans have no waiting period for maternity benefits. Essential Plan has a 6-month waiting period before you qualify for maternity benefits which is much less than other International student insurances with 12-month waiting period.

    【International Student Medical Insurance】uses United Health Care PPO Network, one of the largest medical networks in the U.S.
    Only Essential Plan have 6 month waiting period for pre-existing conditions, but Plus and Elite do not have waiting period.

    If you were previously covered by a U.S. health plan that issues you a Certificate of Creditable Coverage, will credit you for this prior coverage. The number of months of coverage shown on the Certificate will reduce or eliminate the 6 month pre-existing condition waiting period. If you have six or more months of creditable coverage, your waiting period will be eliminated. If you have less than six months creditable coverage, your waiting period will be reduced by the number of months you had creditable coverage. For example, if you have two months of creditable coverage, your waiting period will be reduced from six months to four months.

    No, 【International Student Medical Insurance】is not a guaranteed issue plan. Each application is medically underwritten. Your application may be 1) accepted, 2) accepted with a rate increase due to your health status, or 3) denied.
    The underwriting department will review the application and respond in writing within 3 – 5 days. This may means that we send a request to the applicant for additional information. The turnaround time in these situations will depend on how quickly the applicant responds to the request. If you are applying coverage for yourself only, you are eligible for our expedited underwriting process.
    Under non-direct pay services, you will receive the check for any eligible expenses within 15-30 business days once the insurance company received your claim documentation. 【International Student Medical Insurance】 is under United Health Care PPO with big networks, and it is contracted with lots of providers and hospitals. Under direct-pay services, your responsibility is your deductible only, since the providers and hospitals will communicate with the insurance company directly for billing.
    During your coverage period, 【International Student Medical Insurance】 does not cover in your home country but any other country during your holiday break including travel worldwide. And does not offer any rider covered in your home country.
